A Clinical Study of the Bone and Joint Tuberculosis in Children



Abstract
We have summerized the 109 children with the bone and joint tuberculosis who were hospitalized and controlled at Mokpo Children¢¥s tuberculosis Hospital during the period of 6 years and 9 months from Jan. 1973, to Sept. 1979.
The result were obtained as follow
1) The bone and joint tuberculosis was 10. 9%, out of total tuberculosis children during the above period.
2) The high incidence of age was between 1 and 6 years, which was 63.3%, out of the total patients. Boys were affected more than girls: the ratio of male to female was 1.32: 1.
3) The majority of involved sites was found in the spine which was 80.7%, out of the total bone and joint tuberculosis, and the lesser common sites were hip joint and knee joint.
4) The chief complaints on admission were vertebral bending (38.5%) and the next rank walking disturbance.
5) The most common type of spine tubenr closis was thoracic tuberculosis and the most common involved vertebral lesion was 12th thoracic vertebrae. And 46.8% out of total spine tuberculosis was involved 2 vertebral simmultaneously.
6) In bone and joint tuberculosis except for spine tubeulosis, the left side was involved more frequent than on the right.
7) 85.3%, of the cases revealed strong positive tuberculin skin test with 19.6 mm in diameter of induration.
8) 18.3%, of the cases had a probable sources of infection in the family, and father was found to be most common.
9) The most frequent complication, founded on chest P-A, was pulmonary tuberculosis. (54.1%).
10) In 12cases (11.0%) out of total 109 case, AFB was founded by culture of discharge.
11) Treatment was done with triple therapy (INH, PAS,SM) from Jan. 1973 to Jun. 1976, thereafter Ethambutol ¢¥and Rifampin were substituded for PAS and SM.
12) Average stabilizing period of involved bone and joint on X-ray was between 6 month and 9 month. (50.7%)
